I am aware that providing consent is voluntary and I hold the right to withdraw my consent at any time.

STAGE 2: FORM FILL UP

How to Fill the Form Step-by-Step

  1. Verify Aadhaar Spelling: Check that your child’s Name, Date of Birth, and Gender match their Aadhaar card and school register.
  2. Fill Student & Parent Details: Enter the Parent’s Name, Relationship, Student’s Name, Class, Section, and Parent’s Identity Proof type and number in clear block letters.
  3. Sign & Date: Read the declaration, sign in the parent signature box, and write the date and place of physical consent.
STAGE 3: SUBMISSION

How to Submit & What Happens Next

  1. Physical Submission: Hand over the signed A4 form (along with a photocopy of the child’s Aadhaar card, if requested) to the class teacher.
  2. UDISE+ Verification: The school Headmaster signs the bottom verification box and updates the consent status on udiseplus.gov.in.
  3. Receive 12-Digit APAAR ID: Within 3–7 days, you will receive an SMS confirmation and can download the APAAR ID card PDF in DigiLocker.
DATA PRIVACY & VOLUNTARY STATUS

Is the APAAR ID Consent Form Mandatory?

For minor students under 18 years of age, signing the consent form is mandatory if you want the school to generate an APAAR ID, because schools cannot process a child’s Aadhaar data without verifiable parental permission under the DPDP Act 2023 and Aadhaar Act 2016.

However, APAAR ID creation itself is voluntary for school students. If a parent chooses not to give consent, the school simply marks “Consent Declined” in the UDISE+ portal, and the child cannot be denied school admission, board exams, mid-day meals, or report cards.

WITHDRAWAL PROCEDURE

How to Withdraw or Revoke Parental Consent Later

Parents have the legal right to withdraw consent at any time by following these steps:

  1. Write a formal application addressed to the School Principal / Headmaster requesting withdrawal of APAAR ID consent.
  2. Mention your child’s Full Name, Class, Section, Roll Number, and 11-digit UDISE+ PEN.
  3. Submit the signed request to the school administration office.
  4. The school’s UDISE+ Nodal Officer will update the student’s consent status in the UDISE+ portal accordingly.
